Severe heart failure

Hello,

I have a small dog who is over thirteen years old. Two years ago, we found out he had a heart murmur, and about a month ago, I noticed that Capucin was losing a lot of energy and coughing. I called the vet, and the diagnosis was that he had fluid on his lungs. We changed his treatment to the following:

Nelio 5mg: 1/2 tablet in the evening
Vetmedin 5mg: 1/2 tablet morning and evening
Spironolactone 100mg: 1/4 tablet in the morning, plus 1/4 in the evening for a fortnight.

His cough has eased, but he still has coughing fits several times a day. Strangely, he can seem full of beans one minute and then completely wiped out the next. He is eating and drinking normally, but his urine has a very, very strong smell. He also has a hormonal issue, so his hindquarters are completely hairless with a few sores that sometimes ooze a bit; however, a blood test didn't reveal anything in particular.

The question I am asking myself is: is it normal for the little lad to still be coughing despite the treatment? Is the vet perhaps just trying too hard to keep him going? Is he still happy and comfortable in himself?

As odd as it sounds, he seems much less bloated since starting the treatment. Could the fluid have spread to the rest of his body?

Thank you in advance for your replies.

🆘 Hi everyone, I really need to hear your experiences please... I’m absolutely devastated 😭

Has anyone else dealt with severe bouts of breathlessness that can happen even when they’re resting, along with collapsing and their legs giving way? It happened three times in a single week. Apparently, it was severe heart failure.

How did your dog cope with heart failure like this, especially when it leads to pulmonary oedema (fluid on the lungs)?

I lost my boy during the third episode. The vet told me, "You can come in, but there isn’t much more we can do." I’d already seen the vet twice that same week.

He’d had an operation in mid-January for a benign tumour. He was 9 years old. Like all BCs, he was just the loveliest soul 💔

    Hi there,

I completely understand what you’re going through, and my heart goes out to you. My dog is also getting on in years and suffers from heart failure too; he’s on Vetmedin 5mg, just like yours. This medication has really helped improve his overall condition and reduce fluid retention. It’s worth bearing in mind that heart failure can cause fluid to build up, which leads to symptoms like coughing, even when the treatment is being followed correctly. The fact that he’s still coughing, despite being on his meds, is unfortunately quite common. Vetmedin and other drugs help manage the condition, but some symptoms can linger, especially if the heart failure is severe.

As for the question of whether the vet is ‘going too far’ to keep him alive, I’d say it’s vital to trust their judgement. Vets make decisions based on what’s best for the animal, always weighing up the pros and cons of any treatment. Sometimes, despite our best efforts, the health of our older companions can fluctuate. I’d suggest following your vet’s advice closely, but also keep a close eye out for any signs of distress and make sure he’s as comfortable as possible.

The hair loss and the sores on his hindquarters could be related to a hormonal issue, which is also common in older pets. It’s good news that the blood tests didn't show anything abnormal. As for the smell of his urine, it could be down to changes in his metabolism caused by the medication, so don’t hesitate to mention it to your vet just to make sure there aren't any further complications.

Wishing you lots of strength, and I hope your dog feels a bit better very soon.

My Golden was on Vetmedin from 2008 to 2024 and Prilactone—which is a diuretic to clear pulmonary oedema—for the last year. She passed away last month at the age of 16. She was also on furosemide 40mg and benazepril 10mg. I’ve got loads of it left.

Good evening, I often use homeopathy for my dogs but I wasn’t familiar with Kalium Carbonicum. My question is: do you give it as a supplement alongside their regular meds all the time, or is it only during a cardiac cough flare-up to prevent oedema? Thanks.

Try calling the RSPCA or other rescue centres. Explain your situation to them; they have hardship funds and vets who can help you get your dog treated for free or next to nothing!

Unfortunately, I’m afraid those charity clinics only really help homeless people or those in severe financial hardship. They simply can't help everyone who reaches out to them.

Hi, my little 6-year-old Chihuahua has a heart valve that’s pretty much failed, which is causing pulmonary oedema. He's been on treatment since June and I followed your advice to try KALIUM CARBONICUM 5c twice a day. Despite being on Cardisure, Nelio and Libeo, he’d been coughing a lot for the past fortnight, but for the last three days the hacking and coughing have stopped. Thank you so much!

Mine has been on Libeo, Benefortin and Vetmedin for two years now. The cough started up again a week ago. I’ve got an appointment this Thursday with my vet, who’s looked after him since he was a puppy, but I’ve been thinking about Kalium Carbonicum too, to try and stop this coughing.

I’ll ask my vet if they can recommend a homeopathic remedy, otherwise I’ll go straight to my pharmacist, but she isn’t familiar with my dog's conditions (heart murmur, pulmonary oedema). Hi, my Chihuahua has heart failure, a heart murmur and pulmonary oedema. I was wondering what your vet recommended for yours? I’ve got an appointment on Thursday for a check-up.
